embedded form does not submit responses

Hello,

I tried embedding a form using third party publish options via wordpress.com. The form successfully responds when filled out. However when the submit button is clicked, the button only shows "Please wait..." and does not load the thank you page.

When I checked submissions, nothing is submitted.

I have tested the form you have provided and it's working as intended.

Unfortunately, the link you have supplied to your website seems to not work.

Could you please supply us with the right link to the page the form is embedded on so that we may find inspect what the problem might be.
