How do I create or do you have a template for an invoice submission, not requesting an invoice, but to submit one?

I need a template for invoice submission. It is for a weekly invoice submission to the same place. I just need to change the details of the days of the week and the costings for each day. It should include a grand total for the invoice too. I do not need it to calculate vat. Thank you.
