Mobile responsive template

    Boa tarde, 

    Troquei o template do "card form", mas o template foi alterado apenas no modo phone. 
    Mesmo depois de ter trocado o Template, o template não alterou no modo Tablet e desktop.

    O que faço para alterar o Template em todos os Modos de exibição?

    I apologize for I was not able to fully understand what is the question due inconsistency in the translation to English. Can you please reiterate the question and provide more details.

    From what I got so far, I think you are saying that the card form layout is not working if used on desktop browser. I did test on your form and it appears to be working at this time. The form is in card form layout on desktop browser.
