Account is autosuspended

The email is verified but it is still auto-suspended ? i tried two different emails but the problem still occur ... any solutions ??

As I checked your account, It seems there are email/password fields on your form #81048017585559 (Ayn Ashyaee ?) and the form is suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with the probability of being used for phishing activities. 

Please note that authentication forms can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again.
