    Hello,  I've been unable to log in or reset my password.

    You are not able to log in to your account because it has been suspended. I see that you asked for driver's license number in this form(53206896740158). Please note that it against to our terms of use.

    If you really want to collect this information, you need to provide an official proof. If you have, you can send your proof to support@jotform.com.

    If your proof is accepted, you need to use these widgets in your form:

    1- Terms & Conditions or Short Scrollable Terms

    2- E-Signature or Smooth Signature

    Otherwise, we have to delete this field form your form in order to reactivate your account. We will be waiting for your reply.
