When submitted, the form does not print out the quantities and prices that are totaled on the form here:
It prints all the information above the "My Products" line, but doesn't print the individual products chosen or the prices associated with them.
However, they do print when submitted from here, (which is just the bottom portion of the combined form):
Any ideas as to what I am doing wrong?

Can you please try to re-embed your form using an iFrame code, and see if this will work for you? To get your form's iFrame code, please follow the steps below:
1. Go to Setup & Embed tab
2. Click Embed Form
3. Click iFrame
Just let us know if this doesn't work so that we can further assist you.

Thanks very much, but that didn't help.
None of the items (Products) chosen in the form, nor the amounts are printing.
It only seems to print half the form. Perhaps I created the Product fields incorrectly?

Can you please specify if you are referring to JotForm's print to PDF function? or the actual 'print form' button on the form itself?

Also, I have notice that your integration with paypal on that form is not working properly.
When testing, it is not redirecting the user to the Paypal gateway.
You may want to re-integrate that field and let us know if that helps!

Thanks very much. I don't have any print button on the form. I'm referring to when the form is submitted (to my email address), everything except the bottom part is returned in the email. The part of the form that lists which products are being selected, how many of each and the total, is not being returned in the email.
There should be no PayPal integration. It is a "Purchase Order: form. We just collect the information and then they pay via another gateway on a different page altogether.
Does that make sense? I hope I am explaining it properly.

It seems that field(product field) is not included on your e-mail notification.
