I tried to log into my account and noticed it has been suspended again...why? All of the credit card fields have been removed.

Your account with the username SistersofNiaInc is active and the forms are working correctly.
I noticed that there are two Disabled forms in your account. If you want to enable those forms, please follow the instructions in the image below.

You were using plain text fields to collect credit card information on this form. http://www.jotform.us/form/80507670031145
I have removed the faulty fields and reactivated the account.
If you want to collect payments through your forms, you must add a Payment Integration.
