I thought I selected the option saying payments are required for submission. However, I only have one payment out of six submissions.

I have checked your form and I can see you're using PayPal, please note that your users are indeed redirected to the PayPal payment page; however, once on PayPal they are not forced to pay and they may simply leave the page without completing the payment.
I would recommend you to add some sort of instructions on your form indicating they need to complete the payment on the PayPal payment page.
Also, incomplete payments do not charge the user when you manually complete them, the users will need to send you the payment from their accounts.
