I own an operate an ecommerce site where I sell high end bicycles that often times require me to capture basic body measurements from athletes so I can analyze and recommend the best fitting bicycle. I need help creating a form that could capture this info as well as the persons name, email, phone number and any additional notes. Here are links to a competitor site utilizing a highly customized page. I do not need anythinh this fancy for now.
Take a look at these 2 links and I would like to know if you could help me build a custom form where people could provide me basic body measurements and submit via a form so I can analyze the measurements and help them select a bicycle that will fit them.

If you're looking for something that can calculate entered values and consequently categorize or recommend a fit for certain sizes, then I am afraid to say that JotForm cannot do it.
We currently do not have a calculation feature, so the results cannot be dynamic. However, if the recommendation is manually done, then this is pretty much doable.

Second question. I just launched this form id 22994478007160 and need to
make sure the submitted names are going to my Constant Contact database
names SeaSucker Contest. Can you verify I integrated this correctly into my
account. This way I can capture the contest submissions.

Upon checking, I can confirm that the form was integrated properly to Constant Contact. Have you tried doing a test submission to your form to double check?
Don't worry about incurring a lot of test submissions. You can always ask us for a reset, provided that those are real test submissions (don't delete them so we'll know).
