Form not allowing some users to submit

    We've been using this form for over a year, but recently some users have been complaining that even after they fill out everything required, they are not able to submit the form. One of them printed the form and scanned it and e-mailed it so the application could still be sent in, so I'm attaching what she saw. When she went to sign in, for some reason a logo popped up, which is shown in the attachment. It wouldn't let her sign, but on the scanned form, she signed the paper, so that's the signature that you will see. 

    I have also noticed that when I filled out the form to test it, sometimes when I'd type in a required field and then change it, it would tell me that it wasn't filled out even though I had content in the required field. Refreshing the form and starting over often helped. I'm using Chrome and the person who filled out the attached form was using Chrome. 

    Any thoughts on why it's glitching?

    As i check your JotForm, I see that there is no logo displayed on the form. Did you embed the form on any of your webpages and experiencing the issue? If so, please provide us with the web page URL where the form is embedded so that we can take a look and assist you further.

    Hi, thank you for your response. The form is not embedded, but linked directly. The website is http://richlandonemiddlecollege.com and under the "admissions" menu item is the sub item "application." The application links directly here: https://form.jotform.com/60993760511155

    I cannot replicate the problem myself either. It is apparently happening to just a few visitors.

    Since you mention that the issue could be with some users, could you ask the users to open the form in a separate browser to see if that helps?
