How do I create a form but use another web address?

I have used jotform in the past and used the web address provided; however, I now have a permanent web address that I would like to use instead.  Please advise how I can direct my jotform, once I develop, to use my web address.  Thanks!

The only option for your form to inherit the address of your page is to embed a form.
