You can still edit your form even after setting up the payment. Your form is very long, so it's going to be messy to edit. You can either try to inject some CSS to change the layout, or edit the source code directly. Since you have so many options on your form, injecting CSS of each of them will be a big task. I'd recommend copying the source code into some WYSIWYG editor, such as DreamWeaver, and then rearranging the layout. Or, if you don't have an web editor you could try editing the source directly - maybe by placing the whole form into a table, with multiple options per row.
Here is how you can get full form source code. Open your form on the Form Builder:
1. Open "Setup & Share" tab on toolbar,
2. Click on "Embed Code"
3. Click on "Source" button and copy it to your web site.
