We have a form in which images must be uploaded and yet only rarely does the form accept them. we have a 1MB restriction on the form and have limited to jpeg/jpg file types but still rarely do images upload.

I cloned and checked the form added in the page url included it appears that it does not have the jpg and jpeg extensions added to the accepted file types.
Please add the extensions in the allowed filed types and then save the form. Just go to My Forms and Edit the form, in the menu bar you will see the Extensions just simply add jpg and jpeg to the list. The multiple file extensions can be separated with a comma.
Please let us know if this works.

Please disregard my previous post, just now I was able to replicate the issue with the jpeg upload files. It's strange that this is happening, what I did with the cloned form is to delete and then re add the file upload field and then it should work.
Please give this a try and let us know if this fixe the issue.
