Why is my account suspended?

    Hello,
    I tried to login into account but was told login failed, suspended.
    Find attached screenshot.
    Please rectify this as soon as possible.

    I have checked your account and it has been suspended due to the passport fields you have on your form http://www.myjotform.com/form/80256166718562

    Please note that this is considered as sensitive information, you will need to provide some sort of proof that this information is required for your business, you will also need to update your JotForm profile email address with your corporate address since this will also help to verify your business.

    Please note that it is not allowed to have a File Upload field on your form requesting for a passport photo scan.

    The faulty field has been removed and your account has been reactivated.

    You can request the passport number if you want to.
    It is necessary to add a Terms & Conditions and a Signature widget to your form if you add a field like that.

    You can also choose to encrypt the form instead of adding the widgets.

    I discovered yesterday that I couldn't login to my account and my form was
    disable again.

    You have the Passport and its Expiry Date in the form. This information is considered sensitive which is against our Terms of Use. As mentioned by my colleague, we will allow this but see to it that you will add some terms and condition in the form related to the Passport such as why you are asking it, what its usage for the application. 

    You also need to add an e-signature field in the form for registrants to sign.

    I have reactivated your account again. Please do the changes as soon as possible.

    I tried to add e-signature as per your sample but on the add form element,
    it shows no widget found
    Also how do add only terms and conditions link which will take registrant
    to my website terms and conditions page instead of scrollabe widget on the
    form

    See the cast below which shows how to add the E-Signature Widget and the Terms & Conditions Widget. You can add a link to the terms on your website using the Terms & Conditions Widget.

    My name is oluwaseun Osewa
    I upgraded my account from free account to bronse monthly of $19 on June
    26, 2018.
    I downgraded back to free subscription account on 24th July, 2018 because
    the registration ended on the 24th July 2018 but was surprised when $19 was
    deducted from account through my Paypal account.
