Why isn't my form displaying properly on mobile devices?

I was able to implement this correctly on a custom Zenfolio page, but the form does not adapt for a mobile interface. When I visit the custom page on a mobile device, the form does not fit to the screen. Instead, I have to scroll left and right to view the entire width of the form. Is there a solution for this so a user only has to swipe up and down in order to move through the JotForm?

The form may be displaying like that due to several reasons, for example, the elements on the page are conflicting when the form is added, they need to be somehow adjusted or indeed the form is rendering incorrectly on mobile. 

We will need to get the form URL as well as the page where it's embedded on so we can further test and find what may be causing this.
