I was able to open the stand alone forms without any problems.

I have checked your Contact Us page at http://www.spiritualpaths.net/foundation/contact-us/
And I was not able to find any embeded code of a Jotform. In order to do that you need to follow this guide so you can paste either the Embed or Iframe Code of the form into the body of that page, probably after the "For registration and general Information please contact:" line

Hi, this reponse are usually directed as alert to your jotform account email too. You can check that.
As for your question,. the web form should work on ipad too. That should not be problem.
As mentioned already, I too could not find an embedded JotForm form in the website you provided.
Can you direct us to the correct URL of the website where we can check your form?
