I noticed that you are using a Custom Sender for the Autoresponder Email.
Please make sure that you have whitelisted our IP Addresses and Domains on your Email Server.
Whitelisting-JotForm-IP-Addresses-and-DomainsAlso, double check the SMTP details.
You can always switch the Sender Email to noreply@jotform.com or noreply@formresponse.com.
