I receive a 'page cannot be displayed' when I try view my StaticHTML page with the form on, on a mobile device.
Do you know why this may be?

Can you confirm if you are trying to view the form on a mobile browser in facebook.com or in a facebook apps in a mobile device?
Can you share to us also the URL of the facebook website where we can view the form?

I am trying to view the form on the Facebook App on a mobile phone.
The URL is http://www.facebook.com/MightyDeals.co.uk

From what I can see, it looks like you don't even have your form set up to work with facebook as currently all I see is just the Image that is powered by StaticHTML.
I don't really see anything linking the above Image to your form so this may be leading to the reason as to why its not working. It's not even working for me after I tested it in Chrome. Please refer to this guide http://www.jotform.com/help/22-Adding-Form-to-Facebook to make sure that you've set it up properly.
I'm going to do some further testing on this to see if I can help get it to work for you and either myself or one of my colleagues will get back to you.

Another thing that just came to mind is on the StaticHTML page how do you have your page setup for regular and then fan views with this?
