Form is not pre-populating fields

I have a form that should push values out to a second form.  It looks like it is pulling the information, but not populating the fields.

It's pulling the correct information but the form fields are blank.

Please note that the URL parameters are case sensitive. (managersName2 instead of managersname2 and managersEmail2 instead of managersemail2)

Use this URL instead.

https://form.jotform.com/81333330102135?managersName2={ManagersName}&managersEmail2={ManagersEmail}

The fields should pre-populate correctly now.
