After clearing the form cache, it no longer loads on Chrome and IE.

I just send a message but forgot to send our url: https://atb-therapie.nl/aanmelden/

After Jotform emptied the cache last week it is impossible to open the form in google chrome and internet explorer. Using Firefox the Jotform logo is seen on the bottom of the form.

Please help as clients cannot reach our practice for help.

I did tried to clear your form cache once again, it seems to have fixed the issue. I can now view your form.
