    I have attached a picture of the table I need to configure.

    Based on the patient's score I need to be able to select one of the fields that will be highlighted on the completed form.

    Try the multi-select grid widget. You can modify the selection by editing the submissions https://www.jotform.com/help/325-How-to-Edit-Submissions-Made-on-Your-Forms.

    I hope this helps, but let us know if you have any questions or if you need further assistance.

    It seems there is no option to set values to cells of this widget for now. However, you can add content to the cells using CSS.

    How to Inject CSS Codes to Widgets

    For example, I have filled the table as seen below with using this CSS code by changing the row-column numbers ( yellow highlighted numbers):

    #tablegrid > tbody > tr:nth-child(2) > td:nth-child(2):after {

    content: "cell1";

    }

    #tablegrid > tbody > tr:nth-child(2) > td:nth-child(3):after {

    content: "cell2";

    }

    #tablegrid > tbody > tr:nth-child(3) > td:nth-child(2):after {

    content: "cell3";

    }

    #tablegrid > tbody > tr:nth-child(3) > td:nth-child(3):after {

    content: "cell4";

    }

    Sorry about that. Looks like the account was having the privacy option enabled which is not allowing to clone the form. Please try using the following URL to clone the form.

    Also, please note that the values injected through CSS in the Multi-Select widget will not be displayed in the submission data. The submission data of the Multi-select widget reads with the combination of Row and Column headers as shown below:

    Hope this information helps!
