Have you tried clicking My Froms and checking the forms that you have using the same browser that you have used to create the form? Under your guest account, there was a submission.
You may also check your drliz@ email address including inside the spam folder if you received a notification.
May I suggest as well that you create your own account with us for security and better support purposes.
