Change classic form to one card form

Hi,

I would like to know if it's possible to change the existing and active form layout form from classic one into one card without changing the html address.

Can you explain more your question, please? 

If you talk about changing form layout without changing URL address, yes, it is possible. 

You can do it as the following below:

1) Select your form and click Edit Form 

2) Go to Form Settings that below the Settings tab

3) Click Show More Options button

4) Change layout to Single Question in One Page
