Form does not show up on chrome

we have a form from jotform in our bigcommerce store, under a tab on all the product pages, the form is not there on chrome. any way to fix? 

look on firefox, or IE and its there. 

and if you have any suggestions on how to make it look better, all ears!

I see that you are using the JS embed script of your form. May I suggest that you try embedding your form using its Iframe Code instead, please?
