My form is not working (Sorry! Only one entry is allowed.)

    Several times the form has quit working. I've done my best to repair it. As a very small business, I have only 1 form on your site with a minimal of use. It is still a very important form to us in that it provides us with critical information on our clients.

    One of our clients used the form and got the screen shot submitted. IF we should go over 10 forms a month, I would be very willing to pay for additional submittals.

    Is anything wrong with my form? Can you help me fix it? Thank you.

    This is because you have set the Unique Submission option in your Form SETTINGS to "Check cookies only".

    This will prevent multiple submissions from the same browser.

    If you want to allow multiple submissions from the same browser, set that option (Unique Submission) to "No Check".
