Collecting social security number and drivers license number on our form

I have contacted my lawyer regarding this matter of creating the terms and conditions page and a signature page and what verbiage needs to be written so we comply with your business. With this being a holiday weekend, I will not hear back from him until the end of May or the beginning of June. Thank you for making us aware of this and I am working to rectify the pages.
