Help needed in adding terms to our form

Is it YOUR terms and conditions you want on the page or MY company’s terms and conditions. If it is yours then can you send me the link to import the verbiage you want displayed? 

Do you need me to take those two items off of my application until this matter is resolved?

Please note that JotForm is only a medium to collect the responses from your users for you. You should be adding your company's terms and conditions on the form since your users need to agree to the terms of service that you provide and sign on the form.
