I created a form using your form builder, all ok and when the form is submitted the results are sent to my email address.
But there is one problem?
All the results are coming as lines and the headings are not displayed. As I created a multi page form, all the results are mixed up.
How to group them under each heading as they appear on the form itself.

I've just edited your notification email to include your headings (which I took from the form). You should start to see emails with the correct headings now. If you wish to make further changes, here's how to do it:
Open your form in the Form Builder and click Email Alerts.
Select the Notification (this is the template that you've already created)
The notification can be edited directly in the text pane that opens up. Just find the correct place and make any edits you wish. So for example, in this case I just inserted a new line by hitting enter, and then pasted in the heading from your form.
