hi!
I’ll be providing courses on Facebook through live video,
then I’ll be posting a link to your course evaluation form.
after that I’d like the person submitting the form to receive a generated course completion certificate template with their name on it to their email
is that possible?

Hi, Jotform have several form templates that you can use and/or modify it, you can find them here:

If you want to make a course evaluation form, I would suggest that you take a look in this template:

In order to send the course completion certificate for those who fill your form, you can attach the PDF file of the course certificate to the email auto-responder, you can customize your PDF file (i.e: the course certificate) following this quick guide:

This is what I got going so far.
the person who fills and sends a form, receives this email with an email content at the top, and the pdf content exposed at the bottom. is this the correct application of what you advised me? or should I also explore attaching a pdf file? like on my second image below. Thank you

What you did to the email autoresponder in your form that is shown in your second screenshot is the correct way of attaching the PDF copy of a submission to the email sent to your respondents whenever they successfully submit your form.

Hi again! it seems that it was just the way my iMac displays it. it shows like a regular attachment on a windows pc, and on iPhone. Thank you so much again. Great customer service! I appreciate your help very much!
