Index Form: Landing page for other forms

    I have several different enrollment forms. I'm wondering if there is a way to create one form and depending on the selection of that form, can it open the correct form to be filled out?

    For example:  

    Selection: Existing Employee, Retired Employee, Cobra Benefits

    Depending on the above selection, I would have them directed to a different form I've set up. I know this can be done within a form, but can it be done with multiple forms?

    Yes its possible to create a landing page form with your choices. You can use radio buttons, or a dropdown field if you prefer.

    For example:

    Then using conditions to redirect the user to the chosen form on submission.

    exactly what I'm looking for but I can't find the conditions that say "redirect" anywhere other than the "thank you" page.  Is this where I would do that?

    You shouldn't be creating pages at all. Just one index page with your choices.

    And you shouldn't be using a multiple choice field. You need a single choice field. Or a dropdown field. So that only one form will open (you cant open multiple forms).


    Each choice should have a condition.

    And you will need to get the Form URL for each redirect condition.

    For instance one of my choices is New Employee...I'd like that to go to another page for a separate selection of Benefit start date...depending on the month selected will determine which form gets opened.

    Oh I see. If you want to use page skip conditions you should still use single choice fields. As you can only skip to a certain page (not multiple pages).
