When someone clicks on submit form, he is sent to a blank page instead of my custom thank you page and email notifications are not being sent. What shall I do?

I have the same problem. email notifications are not being sent, and custom thank you page is not sent.

Your form seems to be working fine, I was able to see your Thank you Message.
