Trying to create Incident Report Form with Management Follow Up

    We have a lot of field staff that are having difficulty dealing with a fillable .pdf format so I am hoping JotForm can help us out. 

    I need to design an online incident report where one of our field staff can fill out a portion. 

    When they are finished filling out the first half, it sends the form to the management staff to complete the followup portion.

    I see the "Approval Process Workflow" document and I am not sure that this is what I should be using.  

    Help please?

    Due to conditions not being functional in fillable PDF forms, the approval workflow would not be functional.  Another thing I see in the form you are working with is form collapses and page breaks.  Those also do not translate well to the fillable PDF forms.

    The default notification email would send, so if you only need it to initially be sent to a single address, that would work.  Then you would just send an edit submission link in that email:

    And the manager could finish the second portion of the form from the edit submission link.
