Could you please tell us what is the form that you were working on and that is integrated with Spreadsheet?

Is it this one: http://www.jotform.us/form/71636175997169 since that form is not available now?

    Please advise.

    Thank you!

This is our instructor application form. I reintegrated it with another spreadsheet because we needed the information now...

Were you able to make it work?

If not please let us know what is the other form that you're working on so we can check.

    Thank you!

Whenever re-integrating Google Spreadsheet to your form, it will always create a new file, however, the new file should then have all the submission details.

Can you please confirm if on your new integration the submission details are imported? If yes, are you still experiencing other issues? Can you please elaborate on the issues you are experiencing? Looking forward to your response.

I'm sorry, the re-integration worked. The problem was that the information was deleted out of the original spreadsheet it was integrated with. Here is a screenshot of the original form that was integrated...

Your screenshot did not go through, kindly insert it again in the form following this guide:-How-to-add-screenshots-images-to-questions-to-the-support-forum

May we know if you have changed some fields in your form or in your spreadsheet?

Could you please share the form URL and spreadsheet URL (that is opened for viewing) so we can check this issue further?

    We'll wait for your response.

    Thank you!

That form is integrated to a different spreadsheet now.

    https://docs.google.com/spreadsheet/ccc?key=1yiCNU83bPHfXbqJ_BXPD13o3ayDc_gtxR0NeZv0FKxg

Unfortunately, it is not possible to reintegrate the form with the sheet that was integrated before.

You should be able to restore a version of the original sheet from Google Sheets.

If you want to send the form data from one sheet to another, you might consider using the IMPORTRANGE function.
    https://support.google.com/docs/answer/3093340?hl=en

However, I suggest you keep using the newly integrated sheet now.

Make sure you do not make changes to the sheet or to the form (such as adding/removing/renaming a field) as this might break the current integration.

Ok. Thank you. My original question was why did the form break in the first place. I feel like if the integration can just stop working at any time then that is not good for us. I did edit the spreadsheet, maybe that broke it? However; it did not break immediately or even when another entry was made. It just stopped working, deleted everything and gave me that message. What are your thoughts?

Normally, the integration will be affected if you add, edit or remove a field in the form. There are also instances that if you edit the column headers of the integrated spreadsheet, the integration will stop working. There are also times that our API integration or Google server causes the problem. However, we did not receive any similar reports regarding your issue. So, I believe that this is an isolated case.

If this happens, we usually recommend our users to re-integrate the form to Google Spreadsheets in order to fix the issue.

    Thank you for your understanding.
