How to make a pdf with images

I have designed a form that includes images. I need the client to fill out the data on the form, and have the form (data, images, layout) saved as a pdf and attached to the response email.

These would be "tickets" for admission to an event. The client needs to print the pdf and bring it with them. Last year we told them to print them before hitting submit, which did not work well.
