Hipaa Forms

On regular jotforms within the gold package...can i have a question: Where Does It Hurt? and list different pain points on the question such as knee, back, ankle, etc. as options? Would this be considered hipaa compliant under the hipaa jotform package?

Yes it will be in HIPAA complaint form.

Actually the HIPPA complaint is more about data storage and acess.
