How do I merge forms?

I have several forms with different questions that I would like to merge into one form only. 

You can merge two existing forms by using the Field Manager app, you also can merge the submissions of the form by using the Combine Submissions app. In the Field Manager app, simply select the two forms that you want to merge, and select all the fields from one form and transfer to the other, as you can see in the image below:
