No fields appear on my form

    Hi,

    I have a form that was working fine earlier today and then suddenly it stopped displaying properly. In edit mode everything looks fine, but the live form is not displaying any of the fields. The form link is below.

    Hello,

    I can see that your form fields are hidden with the first conditional statement:

    Please note that those fields will be only shown if this condition is met: How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

    You may disable or fix it for the form to be shown properly.
