Your account has been suspended due to the credit card fields you have on multiple forms in your account:

    1529537802screenshot 01 Screenshot 10

    For example, this form: http://www.jotform.us/form/61046506000136

Note that asking for this information goes against our terms, that's why your account has been suspended, please kindly check our terms in order to find why your account may be suspended: https://www.jotform.com/help/8-Terms-of-Use

It's not possible to allow credit card collection, but if you need to process payments, you may use any of our currently available payment processing apps: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm

Now, in order to reactivate your account I had to remove all the fields from your forms, please also kindly check them and make sure you're not asking for information that goes against our terms, otherwise your account may be suspended again.

    Thanks.

The form you showed was not published and had not been used or even touched I how many months ????
And you suspend the account without a word ???
I will immediately be looking for another service.

Please note that no matter if the form is published, being used or recently edited, if the form has received submissions or even if the form is disabled, we receive an alert for forms that are asking information against our terms and we also have a dedicated team that reviews the forms, that's why your account was suspended.

Also, your account was suspended without prior notice since it's on free status and due to the amount of forms you had asking the same information, it's different with paid users and we do send a notice prior to proceed removing the fields or suspending the account.

This does not mean that you're a phisher, but we need to act according to our terms and make sure our users are safe from those who are really using forms for phishing purposes.

    Thanks.
