The registration form I created fails to work (on page 2) when someone is using a phone.

    I have tested it, and it appears to cut-off, so it is not possible to go to the next page. Please try embedding the form using the Iframe code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code 

    Before you embed the code, paste it in a text editor like WordPad, you will notice two part of codes, only copy and embed the Iframe part in your webpage, example:
