When you attempt to fill the two blanks in the blanks shift, you can't enter into the form. (wrong screen shot attached originally)

It seems you've updated the form but we can investigate this on another form. I'd just like to confirm if that's happening when you use the input mask feature?
My test on your current form is working.

As of checking, I could not reproduce the error.
Note that when using input mask on a required field, the mask must be filled, otherwise it would trigger an error.
