How do I access the data collected from the HIPAA enabled form?

I need a secure contact form for my therapy website.  We want to collect PHI from the form and then have a notification sent to us. 

My understanding is that the data from the form will be hidden in an email notification. How do I access the data collected from the form?  Do I log into my account on the jotform website?

Yes, you will need to login to your Jotform account to view/read the submissions. There is no other way for this when HIPAA is enabled for the account.
