My account is suspended?

Trying to find out why my account has been suspended? Please help! see the link provided. It shows it is disabled at the bottom of the website.

Your account liquid7325 was suspended due to violation of Terms of Use, because the form [Imported Form] rn Snap A Loop Order Form Samplern is collecting credit card details, what is considered as phishing activity:

Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Be advised that you should use one of our Payment Processors to process payments on your form. If all you need is some sort of Payment Authorization (e.g. charge your users at a later date), then your choices would be limited to Paypal, Authorize.net, Square and Stripe.

In order to re-activate your account, we will have to delete phishing fields.
