If you want to allow websites to have the access of your iOS device’ camera and mic, here's a guide:

Step #1. Open Settings app on your iOS device > Tap on Safari. 

Step #2. Now, turn on the switch next to Camera & Microphone Access.

I did this but I am still asked to allow access to my camera for every picture that I am requesting on my survey. So when someone goes to use this on their phone it will make them click “Allow” 10 times.

Each widgets needs authorization.  If the browser requires authorization for one widget, unless you save the setting, each other widget will also require authorization to access the camera.  The setting my colleague referenced should allow for automatic access, but if the browser is still requiring authorization, each one will need to be authorized.
