When the user's make a submission, they are being redirected to another form

Hi there.

We have an injury report that when you hit submit, another injury report from a different organization pops up.  I've rebuilt the page and it still occurs.  (See attached screen shot).

Please advise.  I don't really want to rebuild the report again.

I believe this is the form that you are talking about.

That form has a condition set up to redirect the users to another form on submission.

If you delete that condition, the users won't be redirected anymore.
