My survey form does not load completely

    Once I load my webpage for the first time, the form does not load fully load until I refresh my browser. I have attached a photo as to what happens when I try to load the page for the first time.

    As I check the form on your web page, I see that the Submit button is not displaying completely. Please try embedding the form using iframe embed code after removing the <script> section and adjust the height of the iframe to 900px as shown below:
