How can I put in an URL link in the Hover Text area? My intention is to put a URL link (link to a image file).

To add a link to the hover you just need use and HTML code in it.

I've previously done this and it used to work fine. I've taken another look at the form and it's no longer accepting html.
On the last page please hover above 'Facilities and equipment' file upload option.
Please come back to me asap as this is disrupting our services.
