Calculation

I need to show various shirt sizes with the price to the right and a quantity drop-down menu option, as well as have it calculate at the bottom for a Sub-total price. BUT, I need these options to the right of the picture with the ability to duplicate below each picture.

I'm currently customizing a cloned version of your form and I'll be posting this version in a few minutes.

You can use the Image Checkboxes Widget to stack the images in the same columns and keep the Payment Processor Widget parallel with the images. You can also use the checkboxes below the images to select the products. You can see a screencast of the fixed version of your form below:
