My jot from has stopped showing up on my website

    My jot form that I use on my Website that customers schedule an appointment for is not working. It's embedded into my website just not showing up. We have made no changes to it, and has just stopped working.

    Hello,

    I can see that you form is shown up properly with it's direct URL. Can you please provide us a link to your web page, so we could check it too? 

    I have also cleared the form caches for you, it could resolve the issue. 

    But, please check it and let us know if you need further assistance.
