Single / multiple choice answers are just coming up as blank in my submissions. Are those responses now lost?

    I am sorry for the trouble caused to you. I did test your form by sending test submission and the selection of "Available Backup Mediums" question was captured successfully. Please check the screenshot below:

    I am talking about when you view the form after it has been submitted in the portal.

    Also if you export the form before submitting (ie. print to PDF) the fields disappear. Equally if you click the print in the portal the fields disappear (eg. the first star rating field or available backup mediums field).

    Basically the fields with tick boxes or image sprites dont work properly.

    Did you edit or delete this field (Available Backup Medium) before?

    I can see that you removed a Single Choice field (Radio Button) last June 26, 2018. Please take note that if you removed a field in the form, the data associated with it will be deleted as well.

    Here are your options:

    1. Restore the form to its previous version. If you do this, the form will go back to the version before you removed that field. It means that the data associated with that field will be restored. Here's a guide: How-to-View-Form-Revision-History.

    2. You can just edit the submission. Here's how: How-to-Edit-Submissions-Made-on-Your-Forms.
