Please be sure to check the changes on field variables on the form notificaitons. I would suggest you to re-create the form notifications, delete existing and create new.

I have faced the same problem like you.. we have to delete the existing notification and create new notification ... then i got it. try with this.

Yes, recreating your form notifications would be the best solution to fixed such issue like this.
